A|G’s Required Onset/Location Forms & COVID-19 Workplace Policies:
As we return to work during the COVID-19 pandemic, AARON GREENSPAN (A|G) has implemented a Health and Safety Policy to reduce and prevent the spread of COVID-19 in the workplace.
A|G’s Model Release Form:
Clients over the age of 18 may sign for themselves; if under 18, you must have this release signed by a parent or guardian before any such shoot.
Please understand that signing this release does not guarantee publication of any photo across editorial distribution or in general.
A|G’s Required COVID-19 Screening Form For ALL Shoots:
As we return to work during the COVID-19 pandemic, AARON GREENSPAN (A|G) has implemented a Health and Safety Policy to reduce and prevent the spread of COVID-19 in the workplace.
We are taking additional measures to protect all employees, independent contractors, and anyone on set/location participating in the shoot from the spread of COVID-19.
In accordance with such policy, 24 hours prior to any A|G shoot, and again on the day of the shoot, all individuals must fill out the following form to be on set/location.
Anyone who has any symptoms, has had close contact with anyone who has tested positive within the previous 14 days, or has travelled internationally within the previous 14 days, should stay home, and is asked to contact A|G as soon as possible.

As per CAPIC Guidelines A|G Photography requires all contact information of all members of any shoot, at least 24 hours prior to arriving/being on set at any location.
